Form-5 for all employers

The Employees State Insurance Corporation’s sub-regional office has said that as per a notification issued on March 11 following an amendment to the rules, every employer should append in the revised format (form-5) a self-issued certificate in Return of Contribution’ which needs to be submitted every six months.
